The Ayushmann Bharat Pradhan Mantri Arogya Yojana (AB-PMJAY) - historically integrated with the state's MA (Mukhyamantri Amrutam) Yojana in Gujarat - Continues to provide vital medical safety nets. Eligibility Criteria:
Eligibility is primary, determined based on data from the Socio-Economic Caste Census (SECC 2011) National Food Security Act (NFSA) ration cards, and recent government expansions.
- Senior Citizens (70+Expansion): Regardless of socio economic status or family income, all senior citizens aged 70 and above are eligible for a distinct ayushman card. they receive an exclusive 10 lakh cover ( or top up if the family is already covered).
- Rural Households: Families living in single room kuthch houses, households with on adult members aged 16 - 59, landless households reliant on manual labor, and SC/ST households.
- Urban Occupations: Specified low - income workrs, including domestic workers, street vendors, construction laborers, sanitation workers, and transport operators (rickshaw pullers, drivers).
- Existing scheme holders : Families holding functional NFSA Ration Cards, Antyodaya Anna Yojana (AAY) cards, or registered building construction workers (BoCW).

Ensure you have digital copies or physical prints of the following documents before inistiating the registration Process.
- Aadhaar Card : Linked with an active mobile number for OTP verification.
- Ration Card : NFSA /AAY /Joint Family ID.
- Identity & Age Proof : Voter ID, PAN Card, Driving License.
- Income Certificate : Issued by Competent Authority, if applicable.
- Active Moblie Number.
Step-by-Step Online Registration & Application Process:
Eligible residents of Gujarat can easily apply online or via mobile application using the official National Health Authority (NHA) portals.
Step 1: Verify You Eligibility
- Visit the official Website : PMJAY Beneficiary Portal.
- Select the "Beneficiary" login option.
- Enter your active mobile number, click "Verifa" and input the OTP received along with the Captcha.
- On the dashboard, select your State (Gujarat), Seheme (PMJAY / NFSA), District, and search by Aadhaar Number, Family ID, Or Name.
Step 2 : Perform e-KYC
- if your family details display on screen, locate your name. Unapproved members will show an option for e-KYC.
- Click on e-KYC and choose your preferred mode (Aadhaar OTP, Fingerprint, or lris Scan).
- Select Aadhaar OTP, generate the code, and enter it to verify. Your live details and Aadhaar Photograph will sync with the portal.
Step 3 : Upload Photo and Additional Details :
- Take or upload a recent passport - sized photograph matching the required matching score percentage.
- Enter supplementary information such as your mobile number, relation to the head of the family, pin code, and sub-district/village details.
- Click Submit.
Stap 4 : Status Approval & Downloading the Card :
- Once submitted, the system or state health agency (SHA) will review the details.
- Approval Typically ranges from instant auto-approval to 10 - 15 days depending on documentation accouacy.
- Once approved, log back into the PMJAY Beneficiary Portal, navigate toyour profile, and click "Download Ayushman Card".
Atlernative : Offline Application Process:
If you encounter technical issues or do not have an Aadhaar - linked mobile number, you can register offline:
- Visit the nearest Common Service Center (CSC), Gram Panchayat office, or an Empanelled Government / Private Hospital.
- Meet with the designated Ayushman Mitra (Arogya Mitra) at the hospital helpdesk.
- Provide your Aadhaar card and ration card for immediate biometric verification and spot registration.
